     Mildred Chaulk was born on 25 September 1897 at Great Haseley, Oxfordshire. She died on 15 September 1970 at Radcliffe Infirmary, Oxford, Oxfordshire, at age 72; She was buried in Ickford Churchyard, Bucks.
